Health Plans

There are three plans available for you to choose from. All three plans include prescription drug and dental coverage, however Quest Diagnostics Lab Card is only included with Plan A and Plan B. Click the links below to learn more.

Quest Diagnostics Lab Card
Only Available with Plans A & B
The Quest Lab Card is included with Plans A & B as a way to save you money on outpatient laboratory tests.

Caremark Prescription Drug Plan
Prescription drug coverage is provided through Caremark for all three plans. However the amount you pay will vary depending on the plan you select.

Delta Dental Plan
All employees enrolled in the health coverage are also enrolled in the dental plan. You have access to two provider networks.

Superior Vision Services Plan
You are offered two vision plans through Superior Vision Services - the Basic Plan and Enhanced Plan. Be sure to read the details about your available Vision Benefits.
